Tippi Suzanne Geron, OTD, OTR/L, CCM: Founder
Tippi Geron is a licensed occupational therapist in Virginia and Washington, DC, with a Doctorate in Occupational Therapy and certification in case management. Currently an adjunct professor, Tippi previously served as a full-time assistant professor and Program Director of Shenandoah University’s Occupational Therapy Doctorate Program, as well as an assistant professor at The George Washington University School of Medicine and Health Sciences.
A staunch advocate for individuals across the age and ability spectrum, Tippi specializes in clinically complex cases, including intellectual and developmental disabilities (ID/DD), mental health, stroke and brain injury, dementia, low vision, and home modification. As a published author, expert witness, and dedicated professional, she founded Continuum Care Strategies (CCS) to ensure equitable access to high-quality care for all populations.
NANCY FAGAN, CMC, Chief Operating Officer
Nancy Fagan is a certified care manager with over 30 years of expertise in the long-term care field. Her diverse and comprehensive background spans facilities management, accreditation and quality assurance, care management, business development, legislative advocacy, and forging strategic legal and financial partnerships. Nancy has a deep understanding of Medicare and Medicaid, helping clients navigate these programs to access the services they need.
A published author and respected expert witness, Nancy is known for her collaborative, team-based approach to problem-solving. Her ability to provide clarity and a calm way forward, even in the most intense and high-pressure situations, has earned her recognition as a trusted leader in the field. Nancy's dedication to holistic, client-centered care drives her work at Continuum Care Strategies, where she applies her wealth of knowledge to support individuals, families, and professionals in managing complex care needs.
