Continuum Care Strategies (CCS), LLC is a woman-owned boutique consulting firm based in the Washington DC Metro Area. CCS provides targeted consulting services to assist professionals and individuals in navigating the long term care options and market for all ages. CCS combines years of experience with subject matter expertise to ensure a holistic view which aligns with legal, life, and financial planning, and optimizes available resources to the fullest extent possible.

TIPPI SUZANNE GERON, OTR/L, CCM
FOUNDER
Tippi Geron is a Doctor of Occupational Therapy and is licensed in Virginia and Washington, DC. Additionally, she is a Certified Case Manager.An assistant professor and Program Director at Shenandoah University’s Occupational Therapy Doctorate Program, Tippi has also served as an assistant professor in The George Washington University School of Medicine and Health Sciences in the past. She is a staunch advocate across the age and abilities spectrum with a special focus on clinically complicated situations, mental health, stroke and brain injury, dementia, low vision and home modification. A published author and expert witness, Tippi formed CCS to ensure all populations have access to quality care.

NANCY FAGAN, CMC
COO
Nancy Fagan, CMC, is a certified care manager and has spent 30 years working in the long term care field. Her diverse experience encompasses a wealth of knowledge which includes facilities management, accreditation/quality assurance, care management, business development, legislative advocacy, legal and financial partnerships, and navigating the complicated arena of Medicare and Medicaid. A published author and expert witness, Nancy takes a collaborative team approach to solving problems, and is skilled at providing a calm way forward, even in the most intense situations.
